Yes. First, you must import all your old posts to your new Medium publication using the "Import story" option, or the Medium API.
Second, the custom domain you're using on Medium must be the same as it was prior to moving to Medium (e.g., blog.yourdomain.com —> blog.yourdomain.com).
We will automatically create 301 redirects from your old links to your new links on Medium.
